- Please note that all approvals this year for LPDC will be done via email. If you are due to renew your teaching license this year you will direct all IPDP forms to be approved to Stacie McClelland. Stacie’s email address is as follows: stacie.mcclelland@crooksville.k12.oh.us
- Please be sure to submit Form A and Form B as well as any semester hours or CEU certificates that you have earned since your last renewal.
